# Molina Healthcare beat estimates and held guidance, yet its profit fell 53% and the stock sank about 5%

> Adjusted EPS of $0.89 beat estimates and full-year guidance held, and after-hours sellers still took the Medicaid insurer down about 5%. The fight over whether Medicaid margins have turned moves to Thursday's 8 a.m. call.

[Molina Healthcare](https://theinference.org/markets/companies/molina-healthcare) reported [third-quarter adjusted earnings of $0.89 a share](https://www.sec.gov/Archives/edgar/data/1179929/000117992926000042/0001179929-26-000042-index.htm) on Wednesday, October 21, clearing the 72-cent average of analysts polled by Zacks, on revenue of $11.08 billion against $10.93 billion expected [2]. It reaffirmed full-year guidance: roughly $42 billion of premium revenue and adjusted earnings of at least $5.25 a share [1]. The stock still fell close to 5% in after-hours trading [3][4], after rising 6.3% in the month into the print, to above the average analyst price target of $209 [6].

## Half a cent on the dollar

The beat sits on a base that has collapsed. Adjusted earnings fell 53% from $1.84 a year earlier; GAAP net income fell 52% to $38 million [1]. The medical care ratio, claims cost as a share of premium revenue, reached 92.9% overall and 93.0% in Medicaid, the largest segment [1]. Adjusted pre-tax margin was 0.6% [1]. At that margin, Molina has become a $42 billion toll road that keeps half a cent of every dollar, and one point of claims overrun erases the year [1].

*Chart: **Quarterly profit slid from about $300 million to a $160 million loss** Line chart of Molina Healthcare's GAAP net income by quarter from Q3 2023 to Q4 2025, holding between $216 million and $326 million through 2024, then falling to $79 million in Q3 2025 and a loss of $160 million in Q4 2025*

*GAAP net income by quarter from Molina Healthcare's SEC filings via EDGAR, through Q4 2025, the latest quarter in the filing series. Molina reported net income of $38 million for Q3 2026 in Wednesday's earnings release, down 52% from a year earlier. [10]*

## Smaller on purpose

The fix management chose was [shrinkage](https://theinference.org/article/molina-s-earnings-fix-is-shrinkage-and-october-21-tests-whether-medicaid-can-carry-it). Total membership ended September at 4.862 million, down from 5.628 million a year earlier [1], and revenue has now fallen year over year for three straight quarters [9][1].

*Chart: **Revenue has fallen year over year for three straight quarters** Line chart of Molina total revenue by quarter from Q3 2023 to Q3 2026, rising from $8.5 billion to a peak of $11.5 billion in Q3 2025 then declining to $11.1 billion*

The retreat is concentrated in Marketplace, the Affordable Care Act exchange business: enrollment fell from 713,000 to 267,000, and segment premium revenue nearly halved to $642 million [1]. The whole market is thinning after [richer subsidies expired at the end of 2025](https://theinference.org/article/washington-switched-off-the-aca-enrollment-machine-molina-is-the-stock-paying-the-bill); more than 4.6 million people had left the exchanges by June, and about a dozen carriers, Molina among them, are pulling out of states [5]. Medicaid premium revenue still rose to $8.2 billion on state rate updates [1].

*Chart: **The shrink is a Marketplace story: segment premium revenue down 46%** Column chart comparing Molina premium revenue by segment in Q3 2025 and Q3 2026: Medicaid up slightly to $8.2 billion, Medicare flat at $1.6 billion, Marketplace down from $1.2 billion to $642 million*

Guidance already absorbs a $1.50-per-share loss from [the new Florida Medicaid contract](https://theinference.org/article/molina-rallied-on-a-6-billion-contract-it-will-lose-money-on-in-year-one) starting in the fourth quarter and $1.00 from exiting the Medicare Advantage prescription-drug book for 2027, while parent-company cash and investments were rebuilt to $608 million from $223 million at year-end [1].

## The Medicaid turn finds a second witness

Chief Executive Joseph Zubretsky said medical cost trend appears to be plateauing and that Molina is "likely observing the beginning of the turn in Medicaid margins," pointing to 2027 rate updates [1]. Hours earlier, [Elevance](https://theinference.org/markets/companies/elevance-health), which covers 8.2 million Medicaid members, raised its 2026 forecast on stronger-than-expected July state rate increases, with Chief Executive [Gail Boudreaux](https://theinference.org/markets/people/gail-boudreaux) calling Medicaid "moving from stabilization to improvement" [7].

The bear reply is positioning and arithmetic: the beat cleared a bar set by a fourth-quarter 2025 loss [4], the rebound plan already targets $25 of earnings by 2029, and a [October 14 Hold rating at $217](https://seekingalpha.com/article/4954077-molina-healthcare-paying-21x-for-a-recovery-that-hasnt-happened-yet) argued the recovery is priced in, with January's state rate letters the real test [8].

## Thursday, 8 a.m.

The call opens at 8 a.m [1]. Eastern on Thursday, October 22, with 2027 rates in hand [1]. Listen for how much of the $5.25 already assumes that rate strength, and for whether Medicaid holds a 93% care ratio once Florida loads in [1]. The next hard reading is January, when the 2027 rate letters land [8]. Until then, both camps are trading the same word: turn.

## Takeaway

Molina's beat cleared a bar set by last year's loss: profit halved, the Medicaid care ratio sits at 93%, and the 5% after-hours drop says investors want January's 2027 rate letters, not Thursday's call, before they believe in a Medicaid turn.
