# Molina's earnings fix is shrinkage, and October 21 tests whether Medicaid can carry it

> Molina already made the quarter survivable by shrinking its worst book, exiting Ohio and California exchanges and trimming its 2027 premium revenue. The October 21 print only has to confirm whether Medicaid and duals can carry a business Molina is cutting down.

[Molina Healthcare](https://theinference.org/markets/companies/molina-healthcare) reports after the close on October 21, and the [options market prices a 14% swing](https://www.benzinga.com/markets/market-summary/26/10/62429217/earnings-volatility-watch-bloom-energy-and-other-11-stocks-are-priced-for-double-digit-swings) on a quarter where [consensus earnings fall about 60% year over year, to $0.73 a share](https://www.earningswhispers.com/article/MOH/20473) on $10.93 billion of revenue. The interesting number is not the print itself. It is what Molina already did to make the print survivable: it is cutting its way out of the Marketplace business rather than fixing it, and the stock's 20% year-to-date run has already paid for the half that works. [1][2][3]

## Three numbers settle the print

Molina runs three books. Medicaid, insurance for low-income members, is the earnings engine, guided to about $5.75 a share at a 92.9% medical cost ratio. Medicare duals, people eligible for both Medicare and Medicaid, ran near 92% with a 1.4% first-year pre-tax margin, enough for management to claim its 2.5% target early. Marketplace, the ACA exchange business, is the drag: its ratio guidance was raised to 90% from 85.5%, implying a $0.75-a-share loss, blamed on HIV and oncology members who stayed on the plan despite a 30% repricing for 2026. ## The Marketplace fix is shrinkage

The street's "2026 is the trough" story rests on a smaller 2027, not a better-priced one: 2027 premium revenue trimmed to about $46.5 billion, Marketplace premiums cut by roughly $1 billion, the unprofitable MAPD business exited (MAPD means Medicare Advantage prescription-drug plans), and Molina [leaving the ACA exchanges in Ohio, alongside CareSource, and certain Covered California markets](https://fiscal.ai/company/NYSE_MOH/news/2026-10-15/molina-healthcare-to-exit-certain-covered-california-markets-in-2027/). That is what the 2027 exits mean for the Marketplace book specifically: a stable 90% ratio next quarter will be read against a smaller, repriced book, so if the ratios hold, it will be partly because the worst members have been exited, not because pricing improved. [4][5][6][7]

## The machinery behind the Medicaid guide

The same week, Molina [paused the rollout of DME Navigator](https://www.hmenews.com/article/molina-healthcare-pause-fuels-concerns-over-tpa-models) in several states, after Nebraska's "brief postponement." DME means durable medical equipment such as wheelchairs and speech devices; Navigator is the cost-management tool run by Optum, [UnitedHealth](https://theinference.org/markets/companies/unitedhealth-group)'s health-services arm, and CareCentrix, a home-care administrator, and it is the third-party machinery behind the 92.9% Medicaid guide. On one device contract, the maker Tobii Dynavox counted roughly 200 out-of-network denials since March; Molina did not answer HME News. [6]

## What Wednesday night decides

[At 24 times forward earnings](https://www.ad-hoc-news.de/boerse/news/nebenwerte/cantor-fitzgerald-keeps-usd-209-00-target-for-molina-healthcare-stock/70331614) against its own 13.4-times median, with 14 of 19 analysts on hold, the market has priced the working half. So watch three segment ratios on Wednesday night, not headline EPS. If Medicaid holds 92.9% and duals stay near 92%, the earnings engine is real and the 2027 rebuild keeps its footing. If Marketplace comes in at or above 90% again, each guidance raise this cycle would signal the repricing still is not covering the cost, and the shrinkage was mitigation, not recovery. [1][3][5]

## Takeaway

Molina's October 21 quarter is less about headline EPS than whether Medicaid and duals hold their guided ratios, and whether Marketplace stabilization is real pricing repair or just a smaller book.
