# Walmart's 1.6 million workers get free Ozempic and Mounjaro for diabetes, not weight loss

> The retailer will hand 1.6 million US workers Ozempic and Mounjaro at no cost from 2027, but only for Type 2 diabetes and only after prior authorization. As Starbucks and peers close the weight-loss door, startups like Ilant Health are selling the gate itself.

Walmart told its 1.6 million US workers on October 13 that more than 250 prescription drugs, insulin and inhalers included, will cost nothing under its 2027 plans [1][2][3][5]. Chief People Officer Donna Morris promised "even more ways to help you and your family live better" in a memo [5]. The famous names on the list, [Novo Nordisk](https://theinference.org/markets/companies/novo-nordisk)'s Ozempic and [Eli Lilly](https://theinference.org/markets/companies/eli-lilly)'s Mounjaro, are free only for workers with Type 2 diabetes, and only after prior authorization, the insurer sign-off step [1][2][3].

## Walmart widened coverage and kept the weight-loss door shut

GLP-1 drugs, the Ozempic class of injection for diabetes and obesity, are the test case for what an employer gate is worth. Walmart still does not cover them for weight loss at all [1]. The gate now decides real money: one in eight US adults takes a GLP-1, brand-name injectables run $1,000 to $1,500 a month, and employers may foot 70% to 100% of that bill [4]. [Enrollment for 2027 closes on November 6](https://www.msn.com/en-us/money/markets/walmart-to-cover-some-glp-1s-inhalers-and-insulin-for-employees-at-no-cost-as-it-expands-benefits/ar-AA2ealI1) [1].

## Corporate America is splitting over the same molecule

Starbucks stopped covering GLP-1s prescribed for weight loss starting this month [4]. Among large employers, obesity coverage fell from 72% in 2025 to 60% in 2026, and 14% have dropped or plan to drop weight-management coverage by 2027, a [Business Group on Health survey of 127 employers covering 8.7 million people](https://www.shrm.org/in/topics-tools/news/benefits-compensation/growing-number-of-employers-dropping-glp-1-coverage) found [4]. Walmart moved the other way, holding its starting premium rise to $1.50, to $39.80 per biweekly pay period [1][3][5].

## Massachusetts shows what the gate does to volume

After the state's largest plans dropped weight-loss coverage, 112,000 fewer commercially insured members filled a GLP-1 for that purpose between 2025 and the first quarter of 2026, [a 52% decline that cut annualized spending by $662 million](https://www.wgbh.org/news/health/2026-09-17/sharp-decline-in-glp-1-usage-in-2026-as-insurance-companies-drop-coverage); coverage for diabetes continued [6]. The steepest drops came at the payers that cut coverage [6].

## Ilant Health sells the gate itself

The layer that decides who passes such gates is itself a business. [Ilant Health](https://theinference.org/markets/companies/ilant-health), a New York startup founded in 2022 by [Elina Onitskansky](https://theinference.org/markets/people/elina-onitskansky), once head of strategy at the insurer [Molina Healthcare](https://theinference.org/markets/companies/molina-healthcare), sells obesity care to employers and health plans rather than to consumers [7][8]. It has raised more than $22 million, including a $15 million Series A led by [Cornucopian Capital](https://theinference.org/markets/companies/cornucopian-capital) in June 2026 [7][8].

## The model routes members and shares the financial risk

Ilant's software reads claims data to find high-risk members, then clinicians match each one to medication, surgery or behavioral therapy [7]. The company takes on downside financial risk, so it can lose money when members do not improve, and employers can buy Lilly obesity medicines at transparent prices through Lilly's Employer Connect program [7]. One named customer is SEIU 775 Benefits Group, a benefits group for union caregivers [8].

## The outcomes are the company's own receipts

Ilant's 15% average one-year weight loss and its claimed two-to-four-fold in-year return are company figures, not peer-reviewed results, measured on engaged members [8]. Eileen Pincay, pharmacy practice leader at the consultancy Segal, says many employers have yet to see significant return on GLP-1 coverage at all [4]. The competition is far better funded: Omada Health, now publicly traded, and Virta Health already sell to employers at scale [7].

## What to watch as enrollment closes

Benefit designs set for 2027, not prescriptions, will decide how many Americans get these drugs next year. Medicare is already running a [bridge program that sells GLP-1s for obesity at $50 a month](https://www.forbes.com/sites/geristengel/2026/10/05/obesity-startup-ilant-health-bets-its-revenue-on-better-patient-outcomes/), started in July 2026 and running through 2027 [7]. Enrollment closes on November 6 [1], and the first reading of Walmart's gate will be Lilly's diabetes volume there in early 2027.

## Takeaway

Walmart is widening free drug coverage for 1.6 million workers while keeping GLP-1s diabetes-only and gated by prior authorization; as large employers retreat from weight-loss coverage, benefit design, not prescriptions, will decide who gets these drugs in 2027.
