IQVIA's guidance raise leans on unclosed acquisitions, and $2.1 billion of inactive trials left its backlog
The beat was clean, but the guidance raise leans on acquisitions still to close and $2.1 billion of inactive trials quietly left the backlog. The 400 AI agents are the more interesting number, and December 1 is where the split gets priced.

IQVIA−0.26% — IQVIA, down 0.26 percent today beat on every line this morning and raised full-year guidance, and the market will read it as proof the drug-trial outsourcing recovery is real. But the footnotes qualify it: about 250 basis points of the revenue outlook comes from acquisitions still to close, and $2.1 billion of trials that will never enroll a patient just left the contract book 12. The organic engine is fine. It is not the engine the guidance raise is describing.
The quarter itself needs no rescuing
Revenue of $4.43 billion beat the $4.37 billion consensus, adjusted EPS of $3.34 came in nine cents over the $3.25 estimate, and free cash flow covered 120% of adjusted net income 13.
Revenue has ground higher for three years, about 6% a year

Bookings are the headline inside the headline: net new bookings of $3.2 billion were up 15% year over year, a 1.2x book-to-bill, and CEO Ari Bousbib called it the strongest clinical quarterly bookings since 2022, which for a contract research organization is the leading indicator that matters 1. His list of favorable demand signals, from biotech funding to trial starts, is consistent with a market that stopped freezing and is now spending 1.
The contract book now has to be refilled by real bookings

The raise is not mostly organic
IQVIA lifted its 2026 revenue outlook to $17.35-17.50 billion, and says 250 basis points of that comes from acquisitions expected to close by year end, against roughly 20 basis points of currency help 1. The midpoint of that range, $17.425 billion, sits essentially on the $17.42 billion analysts already modeled 13. Strip the deals and the raise over what the Street expected is close to zero. Organic growth is running near 6%, not 8% 1.
At the same time, the company disclosed that a review of inactive trials removed about $2.1 billion from its $32.6 billion contracted backlog, with no stated impact on guidance or book-to-bill 1. Management frames that as housekeeping, and it may be exactly that, but a cleansed backlog now has to be refilled by real bookings to keep the growth math working.
The AI number no one is pricing
More than 400 AI agents deployed across clinical and commercial work are credited with margin expansion 1. For an outsourcer whose product is billable human trial coordination, an AI agent is not only a margin tool, it is a substitute for the labor being billed.
That can be wonderful, automation that makes trials cheaper and faster, or it can be IQVIA automating away its own revenue base and calling the savings margin. The release reports the efficiency and not the offset, so the honest position today is that the trade is unpriced.
Watch December 1
The investor day on December 1 is where management has to separate bought growth from grown growth, and say what the agents do to the cost of a trial rather than only to the cost line. Until then, own the bookings number, discount the raise, and treat the AI story as the real one still to be told.
