Novo Nordisk halves Wegovy's US price to $675 and lifts 2026 guidance for the third time
Novo Nordisk raised 2026 guidance for the third time this year and lifted its cash-flow forecast, but from January 1 Wegovy's US list price halves to $675. The quarter's volume is real; 2027's revenue now runs on patients paying cash as employers walk away from coverage.

Novo Nordisk+4.98% — Novo Nordisk, up 4.98 percent today raised its 2026 outlook for the third time this year on Wednesday, and the number that should anchor every model on the stock is not in the guidance table 1. It is $675: from January 1, the US list price of Wegovy halves and Ozempic's falls by about a third, a cut Novo itself says will hit 2027 cash flow 12. The Danish drugmaker built the obesity market on insured injections. It is now rebuilding it around patients who pay the sticker themselves.
The quarter beat on volume, with a pricing asterisk
Third-quarter adjusted operating profit rose 27% at constant exchange rates, helped by last year's transformation costs in the comparison base, and both adjusted lines cleared consensus 13. Reported operating profit still fell 9% at constant rates on DKK 8.7 billion of impairments, mostly the write-down of the inflammation drug ziltivekimab and spare factory capacity 3.
Free-cash-flow guidance jumped to DKK 60–70 billion against a consensus nearer 45 2. The Wegovy pill, launched in the US in January, sold DKK 5.4 billion across eight markets, and the wider Wegovy franchise still takes more than half of new US branded-obesity starts 13.
Jefferies+5.37% — Jefferies, up 5.37 percent today counts about DKK 2 billion of the beat as gross-to-net pricing tailwind; without it, Wegovy would have missed estimates, and a CFO handover, with Karsten Munk Knudsen leaving in December for Galderma's Luigi La Corte, adds a debate the bulls did not need 12.

The employers who funded the boom are leaving
PepsiCo+4.17% — PepsiCo, up 4.17 percent today, Starbucks+4.57% — Starbucks, up 4.57 percent today and Cigna+3.37% — Cigna, up 3.37 percent today have all dropped weight-loss coverage from their own employee plans 4. A WTW survey found 53% of large employers still cover GLP-1s for weight loss, the first decline in three years, and another 16% that cover them now are considering dropping them 5.
Some employers already steer workers to manufacturer cash-pay channels, where Novo's NovoCare sells the pill from $149 a month, and a new direct-to-employer deal with the marketplace Andel extends the same idea 56. Jefferies' gross-to-net finding is the tell: net prices were already well below list, and the January cut largely writes that reality into the sticker 2.
2027 is the trade: pill volume against a halved price
Chief executive Mike Doustdar declined to give 2027 guidance before February and named the headwinds: loss of semaglutide exclusivity in some international markets, pressure in China, and a gross-to-net effect less favorable than this year's 1. He conceded the pill's US prescription curve has flattened in recent weeks, blaming seasonality 1.
His defense of the strategy is affordability itself: volumes, he says, have grown "phenomenally" because Novo "made our products more affordable" 7. Lilly's+0.61% — Lilly, up 0.61 percent today rival pill Foundayo is nonetheless taking a growing share of new prescriptions 1. The question the stock now prices is whether pill volume can outrun the discount before Lilly closes the gap.
The bet Novo is asking investors to make is that the patient paying cash is a bigger market than the insurer ever was.
